A transheart (usually stylized as :transheart: btw if that matters) is an icon of a heart overlaid with the transgender pride flag and outlined in red. It is used ironically to express an extreme liking for retarded goyslop, or to associate anything the speaker dislikes with trannies.

"I Transheart Nigger" is Shirtjak's favorite phrase. It mocks soyboys that worship niggers, or cuckservatives/alt-lite figures who worship based niggers.
